JOB SUMMARY:
Responsible for the administrative and technical work in assessing, goal setting, planning, organizing and coordinating services and support with youth who are transitioning out of the foster care system. Transitional Youth Coaches work in collaboration with our partner agencies, schools and other youth service providers in the delivery of services. To perform the essential functions of the job, the Transitional Youth Coach must be able to use personal automobile, have a valid driver’s license in state of residency, and automobile liability insurance.
Note: Requires flexible work schedule and local travel to meet with and clients.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Performs intake and assessments for assigned clients.
Utilizes assessment results to create a goal plan in conjunction with each member.
Supports member with execution of the established goal plan.
Ensures goals in the goal plan are in alignment with overall programmatic goals (education, employment, positive peer relationships, community involvement and life-long connections).
Maintains appropriate documentation of member goals and progress utilizing recommended tools and software.
Provides hands-on life skill training to members.
Provides information and referral to formal and informal community partners as appropriate.
Assists youth in accessing higher education/vocational training.
Works collaboratively with other staff/partners for the benefit of members.
Meets, consistently, with members with respect to the meeting frequency established in the goal plan.
Attends treatment team meetings.
Assists in arranging transportation for members when needed.
Assists in the coordination/planning of social events for members.
Provides guidance and support to interns as needed.
Represents the program/agency at community events as may be requested by the Director of Clinical Services.
Cultivates and manages positive relationships with contractors, partners, and funders.
Complies with agency policies and procedures and changes in such, as outlined by supervisors, administrators, agency handbooks and manuals.
MINIMUM QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the minimum knowledge, skill, and ability required unless otherwise stated.
EDUCATION and/or EXPERIENCE:
Bachelor’s degree in Human Services, Sociology, Psychology, Social Work, Marriage and Family Therapy or Professional Counseling.
Minimum of one (1) year of experience in the area of child welfare, adoption and/or youth service.
Experience in interfacing with private and public social service systems.
Experience working with transitional age youth aging out of foster care.
Experience in youth development and community engagement.
KNOWLEDGE, SKILLS, & ABILITIES:
Working knowledge of modern theories as they it relate to trauma; grief and loss; attachment; motivational interviewing; and youth engagement.
Familiarity with the implementation of strengths based models of care.
Demonstrated ability to conduct self in a manner reflecting credit on the agency and encourages others to the same.
Skilled in the use of Windows based PC environment and Microsoft Office tools such as Outlook, Word, and Excel.
Ability to quickly learn various software applications and tools relevant to the job; e.g., software specific to social work and clinical environments.
Skilled in the use of office machines and equipment including phones, fax, printer, and copier.
Excellent interpersonal skills including the ability to work with a wide audience of internal and external parties.
Excellent verbal and written communication skills including accurate grammar, ability to write business correspondence, reports and give oral presentations.
Good organizational skills including: working knowledge of efficient filing systems; attention to detail and follow-through; and ability to meet deadlines.
Good basic arithmetic skills including attention to accuracy, detail and follow-through.
Ability to analyze, isolate, and resolve problems efficiently and effectively.
Ability to manage and prioritize multiple and changing demands with diplomacy and decorum.
Ability to establish and maintain effective work relationships with internal and external clientele at all levels of employees and management, existing and potential customers, contractors, vendors, and agencies.
Ability to work in and constructively contribute to a harmonious cross-functional team work environment.
Ability to maintain strict confidentiality of all client and business operations
