POSITION SUMMARY: The Care Navigator will join a team of 5 other Margot Murphy Breast Cancer Program Care Navigators and will be responsible for providing case-management and psycho-social support to approximately 30-40 clients. The majority of clients served are low-income and/or from immigrant communities in San Francisco.
AGENCY SUMMARY: Shanti exists to enhance the health, quality of life, and well-being of people living with life-threatening illnesses. Each year, we serve over 2,000 people living with cancer or HIV/AIDS, many of whom are low-income and medically underserved. We provide culturally and linguistically competent one-on-one Care Navigation, practical and emotional support through trained staff and long-term Volunteer Care-Givers, basic in-home assistance, as well as Health & Wellness survivorship classes and events. For many of our clients, Shanti is their sole support and care advocate during this extreme time of need.
PRIMARY DUTIES:
Provides Care Navigation (case management and emotional support) to cancer-patient clients including: client needs assessments, patient and social services advocacy, individualized care planning, appointment accompaniment, and high-touch referrals to other resource providers within the community
Collaborates closely with partner agencies
Provides emotional support and practical assistance to clients including home and hospital visits
Maintains client files and records monthly units of service (UOS) as required by contracts
Provides support to our Health & Wellness Activities for clients in survivorship
SKILLS AND REQUIREMENTS:
Fluency in written and spoken English
BA/BS or equivalent experience
2-3 years experience in providing direct services preferred
Demonstrated comfort in working with diverse populations
Knowledge of resources in SF community preferred
Fluency in second language preferred
