POSITION TITLE: Case Manager – Somali

DEPARTMENT: Youth

REPORTS TO: Youth Program Manager

STATUS: Non-exempt Regular Part Time (23.5 hours)

PURPOSE: Provide case management and family advocacy services in the Youth Job Readiness Training (YJRT) program

JOB RESPONSIBILITIES

Conduct outreach with community providers, schools, and Somali families, providing information about YJRT and how it will benefit students and their families.
Provide a minimum of one orientation session for students and families.
Develop and maintain relationships with school staff and administration through frequent communications to ensure academic progress and family support.
Recruit students/families for program and complete intake process and paperwork for 17 youth and their families.
Develop Individualized Service Plans (ISP) for youth and families.
Meet with students’ families on a bi-weekly basis (on-site, home visit, school visit) to monitor progress and identify emerging needs.
Develop tailored materials and provide trainings on home-based academic support activities for parents.
Provide internal and external referrals to social services on an as needed and on-going basis.
Communicate monthly with students’ teachers to provide and receive information on student’s academic progress.
Maintain complete, accurate client files and data assessment measures.

Facilitate communication and engagement between focus students, parents, and schools.
Staff workshops for parents and youth that are tailored to support the individualized needs of the families and youth.
Support families by providing information and translation for key school events (i.e. open houses, student-teacher conferences and family nights).

Communicate and meet regularly with the Youth Manager, Program Coordinator and After School Program Lead staff to arrange workshop schedules, discuss and plan programming, evaluate program and client progress, and ensure program objectives and grant outcomes are being met.
Attend all monthly Youth Program Team meetings.

Maintain confidentiality when working with youth and families.
Attend all monthly agency meetings, as well as other relevant City of Seattle, community-based, school district, or provider meetings.
Compile and submit all paperwork including contract reporting, assessment data, timesheets, and other agency forms in a timely manner.
Any other duties as assigned by the Program Coordinator or Youth Program Manager.

MINIMUM QUALIFICATIONS

Associate’s degree in human services field, and at least one year professional experience as a case manager in human services, or at least two years of experience as a case manager.
Successful experience working with immigrants and refugees, adults and youth.
Proven ability to provide quality job support services to clients.
Bilingual in Somali and English.
Demonstrated ability expressing yourself effectively, both verbally and in writing, using the English language.
Excellent organizational and outreach & recruitment skills.
Ability to work independently and as part of a team.
Demonstrated computer skills including knowledge of Excel, word processing and email programs.
Must be able to pass a Washington State and multi-state background check and be able to provide fingerprints for state requirements.
Must have own transportation, a valid Washington state driver’s license and current insurance, and ability to travel to multiple sites on a regular basis.
Willingness and ability to work flexible hours (weekend hours when necessary).
Experience collecting data and writing reports.

PREFERRED QUALIFICATIONS:

Bachelor’s degree in human services or social work.
