Case Planner, Bronx Region – The New York Foundling

We are currently looking for a Case Planner in Bronx Family Foster Care Program.

Responsibilities:

Case planning with goal of permanency for children either through reunification with parents, and if not possible, through adoption or preparation for independent living. Implement three evidence-based models with foster parents, birth parents/discharge resources, and youth which includes running groups and supervising family visits onsite care using an evidence-based approach.

Advise/counsel/advocacy/case planning with parents and/or discharge resources and children. Conduct birth/foster home visits, accompany families to drug treatment, medical, school, housing, welfare setting, and attend court hearings field visits. Provide referrals to community service providers in accordance with the service plan for the family.

Prepare court reports, permanency hearing reports, progress notes, FASP, plan amendments via connections within the required time frames. Attend weekly team meetings, consultation, and supervision.

Work collaboratively in coordinating and providing service with agency/community clinicians, ACS, educational providers, collateral agencies, and co-workers. Participate in all trainings including training for evidence-based models and other trainings to support model implementation. Any other duties as assigned.

REQUIREMENTS: BA/BS or MA/MS in Social Work, Psychology, Sociology or a related degree. Experience working with children and families preferred. Knowledge of community resources and ability to effectively negotiate systems for multi-problem families. Experience with drug treatment populations preferred. Computer literate, good organization and interpersonal skills. Bilingual English/Spanish preferred.
