PROGRAM COORDINATOR- Case Manager
Limited, Full time, benefitted position
The Program Coordinator-Case Manger will do intake, assessment, application assistance, and referral to social services for individuals and families in San Mateo County. S/he will be an employee of the Daly City Partnership, reporting to the Executive Director, and will be stationed at the Daly City Community Service Center office, located at 350 90th Street in Daly City. As one of eight county core agencies, the center assist residents in need with emergency shelter, utility assistance, food, clothing, guidance, counseling, and other basic safety-net services. The Program Coordinator (PC) will provide supervision and training of community case workers, ensuring that the highest level of polite, dignified client service is maintained.
EXAMPLES OF DUTIES
Plan, coordinate and direct varied Community Service center activities
Perform detailed client intake and co-create a goal-based service plan.
Analyze and document client budget and needs for basic emergency services.
Provide information, advocacy, referral and support in the application process to qualifying social services, such as Medical / Medicare, Cal Fresh, Family Harvest, In Home Support Services, Center for Independence of the Disabled, etc.
Input detailed data on client profile, programs and services received into the ‘Clarity’ system on a timely basis.
With client consent, conduct follow-up assessments at regular intervals for continued needs, coaching clients on their stabilization plan.
Train, monitor and supervise staff and volunteers
Analyze the center’s data quality, performance reports, client profiles, programs and services received.
Conduct weekly or bi-weekly staff meetings, and direct staff supervision.
Assess and approve or decline guideline-specific housing applications or funding requests
Monitor budgets, control expenses and meet spending goals for charitable giving
Attend DCP, City and County meetings and trainings
Organize and help lead community service projects ( Backpack Drive, Operation Santa Claus)
Conduct evaluation of services and write comprehensive narrative reports on outcomes.
Conduct outreach regarding core services at community centers, medical facilities, and other public venues.
Establish and maintain harmonious public relations, community connections and partnerships
Participate on community collaborative teams, attend staff and board meetings, City, County, and Daly City Partnership meetings, functions, and fundraising events.
Seek grant funding, assist in program development and write (or assist with) proposals.
Other duties as assigned.
MINIMUM QUALIFICATIONS
Knowledge of:
Non-profit, community, and county services.
Basic client assessment and sensitive case management
Issues facing at risk families, the homeless, and culturally diverse communities.
Methods and objectives of social services and mental health counseling.
Confidentiality issues related to case management, including HIPPA, State & Federal Guidelines
Microsoft Office, including Word, Excel, Outlook. Etc.
Ability to:
Provide strong leadership and support the mission and vision of the Daly City Partnership to guide collaborative efforts and connect the community to services that promote well-being.
Assess, plan, organize, schedule, and provide linkages to community resources.
Communicate effectively orally and in writing.
Establish and maintain effective working relationships with those contacted in the course of work
Attend work and perform duties on a regular and consistent basis.
Maintain appropriate documentation of work, and evaluation of services provided.
Adapt and be sensitive and tolerant of culturally diverse people and their traditions.
Provide patient, compassionate counsel and care to clients on a regularly scheduled basis, including those with diminished physical and mental capacity.
Work occasional nights and weekends, as required.
Lift 30 pounds.
Experience:
Four (4) years of community-based work experience that includes work with economically and culturally diverse populations.
Two (2) years of training and supervision with 2 or more co-workers.
Spanish-speaking highly desirable.
Education:
Bachelor’s Degree in Social Work, Public Health, Sociology, Psychology or a related field from an accredited college or university.
Master’s degree preferred
MFT or MSW internship considered, with clinical supervision provided.by DCP
License:
Possession of a valid Class C California Driver’s license and clear DMV record
Passage of background checks (DOJ, FBI and other screenings).
Must have access to a car for trainings and travel and proof of liability and comprehensive vehicle insurance.
