The FIND FH Initiative
FIND FH is a new initiative of the FH Foundation designed to identify and engage the estimated 1.5 million Americans with FH. FIND FH utilizes advanced data mining and machine-learning technologies to identify possible FH patients within multiple care settings. Various forms of outreach are then used to connect with these patients. The FIND FH Health Care Provider (HCP) & Individual Engagement portion of the FIND FH Initiative will develop and implement a FH Foundation call center to support the identification and engagement of individuals with FH and their HCPs.
Position Reporting to the CEO, the Program Manager will be responsible for the development and operational success of the FIND FH HCP & Individual Initiative, ensuring seamless team development and management, program delivery, and quality control and evaluation. In this newly established role, the Program Manager will hire and manage a staff of 4 over the first eighteen months. The Manager will also be responsible for the development of the HCP engagement processes.

Leadership:
Cultivate existing relationships with all FIND FH team leaders
Develop and implement strategies that will successfully engage HCPs
Team Management and Development:
Develop and implement a system to evaluate the skill, experience, and professional development needs of all staff
Develop, test, and implement HCP engagement processes
Implement a professional development program to address employee experience and skill gaps
Work with staff to develop objective performance measurements across all aspects of the FIND FH HCP & Individual Engagement Initiative, to ensure consistent, high-quality evaluation and performance for all FIND FH team members
Instill a sense of accountability among team members by modeling active oversight of individual and organization performance standards
Recruit, hire, and oversee training and orientation of FIND FH staff members
Program Operational Management:
Develop balanced score card and program dashboard; establish consistent, objective program performance standards of accountability
Qualifications
This is an extraordinary opportunity for an individual with team management experience to grow and further develop a proven program. The successful candidate will lead programs, partner with the CEO and work collaboratively with a high-performance management team. Specific requirements include:
Minimum of a Bachelors degree, advanced degree preferred
Healthcare background and experience in starting new initiatives is highly preferred
At least 10 years of experience, with three of those in a team management role
Excellent verbal and written communication skills with exceptional attention to details
Demonstrated success developing and evaluating program models, and selecting and successfully operationalizing innovative programs
Proficient in using technology as a management reporting tool and experience working with information technology staff to develop and implement program evaluation systems
Strong project management skills managing complex, multifaceted projects resulting in measurable successes and program growth
Experience having worked with a high-performance, collaborative, constructive peer group
Strength in hiring, recruiting, managing, developing, coaching, and retaining individuals and teams, empowering them to elevate their levels of responsibility, span-of-control and performance
Deep understanding of human resources, employee performance improvement plans, and corrective action policies
Demonstrated results in managing through complex systems and proven experience negotiating win-win agreements
Personal qualities of integrity, credibility, and a commitment to and passion for the FH Foundations mission
