Job Title: Employment Services Case Manager
Reports To: Director of Employment Services
Essential Duties and Responsibilities
Conduct program orientations and information sessions, and implement eligibility review process. Implement and conduct trainee employability assessment process. Assist clients in identifying and prioritizing employability, work readiness goals and service needs and mitigating self-sufficiency barriers with an emphasis on social services resources. Finalize worksite training referrals and evaluate the worksite training progress of program participants. Plan, develop and facilitate the provision of services, activities and resource necessary to facilitate, and enhance trainee program participation.
Key Duties
Conduct program employment and training orientation with participants as needed.
Assist in the Work Readiness Assessment Process as necessary
Develop individualized service strategy identifying human services, activities and resource needs.
Assist in the development of the Employment Development Plan with an emphasis on barriers to self-sufficiency; assist participants in identifying and prioritizing social service needs.
Manage the classroom training referral process serve as training support throughout the classroom training period.
Identify support service needs of ES program participants, monitor supportive plans as needed.
Evaluate the progress of each participant in acquisition of the required vocational skills, arrange tutorial assistance, corrective measures, such as remediation and/or extension of training where necessary.
Conduct classroom training follow-up activities, and update Individual Service Plans, and document all required information in a timely manner.
Maintain case notes relative to support services provided throughout program participation.
Ensure participant is provided the resources needed to achieve employment goals.
Basic Qualifications include:
Minimum 2 year college degree (Preferred)
Minimum of (5) years of experience in human services, employment, training, education or non-Profit fields (Mandatory)
Workforce development case management experience (Preferred)
Case management, peer counseling or job coaching (Mandatory)
Computer literacy in word-processor and spreadsheet programs. (Mandatory)
Knowledge of local human service agencies resources requirements, and enrollment practices
Excellent oral and written communication skills.
