ORGANIZATION:

The Community Health Center Association of Connecticut, Inc. (CHCACT) is a not for profit organization committed to primary health care access for all Connecticut residents and to improving the quality of primary care services provided by the State’s network of Federally Qualified Health Centers (FQHCs).

POSITION DESCRIPTION:

The Teaching Health Centers/FQHC Residencies Program Manager (THC) will work within the CT Practice Transformation Network (CT-PTN), a newly funded Centers for Medicaid and Medicare Services (CMS) grant in the Transforming Clinical Practice Initiative (TCPI). The CT-PTN is a statewide, FQHC-centric network that builds upon FQHC Patient-Centered Medical Home (PCMH) recognition, team-based care delivery, integration of specialty/behavioral health with primary care, resource coordination, and population health. The THC Program Manager will be responsible for providing direct support to the THC Director in building affiliations between FQHCs and academic institutions, recruiting and vetting residents for placement in FQHCs, and preparing submissions to the Accreditation Council for Graduate Medical Education (ACGME). Additionally, the THC Program Manager will ensure linkage between the THC project and CT-PTN for transformation training for residents.

REPORTS TO:

Teaching Health Centers Director

QUALIFICATIONS:

Must have a degree in a health-related field.
Experience in a health care provider or academic institution setting.
Understanding of and sensitivity to unique characteristics of non-profit healthcare setting.
Ability to work in team environment and support/contribute to achievement of objectives.
Must have strong computer skills, including MS Office Suite; Excel, Word, Access, and PowerPoint. Experience with Outlook email and SharePoint database is desirable.
Strong interpersonal skills, oral and written communication skills, and ability to analyze problems and recommend effective solutions.
Excellent organizational skills.
Must possess valid driver’s license and provide own transportation for frequent in-state travel.

COMPENSATION:

Full Time (37.5 hours/week) Exempt

Salary: $42,500
