Youth Case Manager (SYVPI) – El Centro de la Raza – Seattle, WA

POSITION SUMMARY

Provides case management services to (27) middle and high school youth ages 14 – 17 who are at the highest risk of perpectuating or being victimized by violence; and who are eligible for Initiative Services in the Southwest District. Case manager commits to achieving results for Seattle’s high risk youth; 50% reduction in court referrals for juvenile crimes against persons committed by youth in Southwest Seattle (Denny M.S. and Chief Sealth H.S); and a 50% reduction in the number of middle school suspensions/expulsions due to violence related incidents.

ESSENTIAL FUNCTIONS

 Provide case management services at school site and at ECDLR to youth referred

 Meet the the goals and objectives outlined in the performance standards (copy will be provided)

 Attend Initiative all-team meetings and Initiative Case Managers meetings on a monthly basis or as scheduled by the City of Seattle-Human Services Department. Complete and intake and assessment process using the SYVPI Risk Assessment Tool and Motivational Interviewing (MI) to determine youth’s needs and strengths,

 Identify goals and develop an Individualized Service Plan (ISP),

 Create a shorterm achievable action plan for each youth.

 Updates Individual Service Plan every 3 months.

 Follow up assessments will be made very six months and/or upon exit from the case management services.

 Provide culturally and linguistically appropriate services that include intake and assessment, ISP development, advocacy, information and referral services, assistance navigating the school, court, social services, employment and training systems. Serve as a role model, advocate and life coach to the youth.

 Creates and updates accurate hard copy and database files/information and maintains complete client chart notes which will reflect the progress of participants.

 Serves as an interpreter for non-English speaking parents.

 Follows up with service providers.

 Creates monthly, quarterly reports and annual reports associated with contractual reporting.

 Develops relationships and networks with other agencies and communities.

 Acts as the liaison between the school and the youth/family.

 Attends staff meetings, case manager meetings and monthly general meetings.

 Performs other duties and responsibilities as assigned; and in accordance with the contract. Attend Charlas, staff meetings, youth team meetings and meetings with the Human Services Director.

JOB QUALIFICATIONS

Essential minimum skills, education, and experience:

 Bachelor’s degree or adequate experience working in the field.

 Previous experience working with at-risk youth a must.

 Must be bilingual Spanish/English.

 Must have a car and a valid Washington driver’s license and show evidence of insurability.

 Must have excellent oral, written and organizational communication skills.

 Must be effective in conflict resolution.

 Must have excellent computer skills, specifically in Excel and Word.

 Familiar with Outcome and Indicator Reporting.

 Must have good attention to detail.

WORKING CONDITIONS/ENVIRONMENT

Works on site and the schools and hold office hours at ECDLR. Works 40 hours per week. Requires local travel visiting schools, making home visits and evening hours Requires a flexible schedule. _____________
