Case Managers are responsible for performing a variety of tasks and activities that are designed to effect the lasting rehabilitation of residents including (1) initial and ongoing assessment of residents; (2) monitoring and evaluating of resident level of functioning; (3) development and review of individualized a service plans with residents; and (4) delivery of supportive services in accordance with the resident’s needs, desires and responsible for working as a member of the services team and for having a working knowledge about the epidemiological of Aids, clinical drug trials, CPR, First Aid, infection control and OSHA guidelines, Medicaid managed behavioral care and CARF accreditation.
Case Managers maintain an active caseload of single adults.
Case Managers report to and are supervised by the Clinical supervisor. In the absence of the Clinical supervisor, case manager report to and are supervised the Program Director.
This is an exempt position.
Responsibilities include, but are not limited to:
1. Participate in the intake of people for admission to the scattered site-housing program and in the initial intake of new admissions.
2. Following development of initial assessment, conduct at least semi-annual assessment of residents included in the case manager’s caseload to determine resident functioning, needs and strengths.
3. Develop initial individualized service plan with residents in accordance with the resident’s desires, needs, and strengths. Involve the resident’s family and other collaterals in service plan development as necessary.
4. Review service plan at least quarterly and revise as necessary in accordance with the resident’s desires, needs goals and progress. Involve the resident’s family and other collaterals in service plan review and revision as necessary.
5. Ensure that objectives and services/methods included in the service plan relate to goals in the service plan and relevant for the resident. Provide skill training as necessary and in accordance with the resident’s desires, needs and goals.
6. Provide direct assistance to residents in daily life skill areas in which the resident may be deficient and requires skills development or the retraining of skills.
7. Record the progress of residents within 24 hours of service delivery and accordance with the individualized service plan. Record the progress of residents within 24 hours of contact with the resident that is not related to the individualized service plan. Record telephone conversations with residents immediately following each telephone contact.
8. Ensure that resident case records are current and up-to-date and that they are maintained in accordance with organization and accreditation requirements.
9. Make referrals of residents to locally based supportive service providers as required, acts as liaison for residents with other service providers, escorts residents to meetings and appointments with other service providers as needed and regularly meets with other supportive service providers to ensure continuity of care.
10. Attend and participate in case conferences at the program site, with the Division of AIDS Services and Income Support and with other supportive service providers as required.
11. Assess the income assistance needs of residents, advise residents of the requirements of financial assistance program(s) to which the resident may be entitled, assist residents in the income assistance application process, accompany residents income assistance office(s) as needed and advocate on behalf of residents to ensure timely receipt of benefits. Keep abreast of changes in rules, regulations and guidelines of income assistance programs.
12. Respond to resident crisis, emergencies and other situations that require immediate staff involvement and actions.
13. Maintain the philosophy that people diagnosed with HIV/AIDS can lead functional and productive lives in the community.
14. May assist the Activity Coordinator in the implementation of on- and off-sit activities.
15. Maintain a positive relationship with residents of the scattered site housing program and encourage residents to participate in the program to their fullest.
16. Collect funds for monthly rent excess utility bills and excess telephone bills from residents and present payments to the program’s Bookkeeper. Ensure that all residents receive a receipt for monies collected and present a copy of the receipt to the Bookkeeper (o Residence Manager) for the program files.
17. Maintain a positive working relationship with locally based supportive service providers.
18. Attend conferences, meeting educational seminars as required by the program Director and/or Clinical Supervisor.
19. Act as an advocate for residents.
20. May participate in the orientation of newly hired staff.
21. May participate in the in-service training of all staff.
22. Maintain a positive working relationship with housing case management staff at the Divisions of AIDS Service and Income Support.
23. Ensure that program operations standard established by the organization including those related to national accreditation and Medical managed behavioral care are maintained at all times.
24. Perform related duties and responsibilities as required by the immediate supervisor.
Minimum Qualifications: Bachelor Degree in a human service field. Related experience and experience working with people with AIDS is helpful.
