Case Manager – Community Housing Partnership – San Francisco, CA

Community Housing Partnership’s mission is to help homeless people secure housing and become self-sufficient. Community Housing Partnership is an outcome focused nonprofit that fulfills its mission by developing and managing high quality permanent housing and providing services to homeless individuals, seniors and families to help them rebuild their lives and break the cycle of homelessness. Community Housing Partnership is financially stable with an annual operating budget of approximately $25 million. The organization has an outstanding staff of over 280 people, over half of whom were once homeless.

During the past four years, Community Housing Partnership reorganized its operations to better manage its growth and to refocus its services on long term impacts and outcomes. CHP is currently undertaking a repositioning of its programs and services to align with a newly developed “theory of change” model that will be implemented over the next four to six years, with the support of a committed foundation. Position Overview:

The Support Services Case Manager provides support services to formerly homeless tenants who live in a supportive housing building. The services provided are voluntary and include housing retention, counseling, information and referral, crisis intervention, community organizing, case management and service planning. Services are provided within the context of harm reduction practices and follow a peer-based model of care.

Responsibilities:
· Provide housing retention services.

Provide case management services to tenants.
Create and track tenant service plans.
Provide activities for community building and tenant empowerment.
Provide services to tenants using harm reduction principles.
Perform tenant intakes and ensure that appropriate follow-up and/or referrals are complete and documented.
Develop and lead at least two activities per month.
Provide ongoing creative outreach and engagement to tenants for support counseling, referral and tenant involvement in building and community events.
Provide crisis intervention, conflict resolution and de-escalation services to tenants.
Assist tenants in preparation for unit inspections and annual re-certifications.
Participate in unit meetings, staff meetings, department meetings and organizational work groups.
Provide back-up coverage for other Support Services staff.
Other duties as assigned.

Qualifications:

Experience providing services to homeless individuals preferred.
Ability to provide effective services to diverse tenant populations.
Experience in crisis intervention, conflict resolution, and counseling.
Experience in organizing groups, group facilitation and group development.
Demonstrated commitment to help break the cycle of homelessness and poverty, and to empower formerly homeless people by involving them in all aspects of CHP.
Demonstrated ability to work with formerly homeless individuals and those struggling with substance abuse and mental health issues and disabilities.
Strong interest in doing social justice advocacy.
Administrative and computer skills (MS Word, Excel).
Desired qualification in fluency in English/Spanish.

Required Education, Credentials & Licensure:

High School diploma or GED required.
Two years of experience in the provision of social services or equivalent experience.

Position Details:

Location: Arnett Watson Apartments, San Francisco, CA
Full-Time / Non-Exempt status
Rate: 17.50/hr. + possible bi-lingual premium
Union: This position is under the collective bargaining agreement with SEIU 1021
Reports to: Support Services Manager
Office Hours: Full-time (40 hours per week), Non-Exempt, Mon. 9-5pm; Tue. 11-7pm; Wed.9-5pm; Thr. 11-7pm; Fri. 9-5pm with some evening and weekend hours.
