TITLE: Alice Griffith Case Manager -Senior Service Connector

REPORTS TO: Senior Project Manager

LOCATION: San Francisco, CA

BACKGROUND:Urban Strategies is a not-for-profit organization founded in 1978, headquartered in Saint LouisMissouri. Urban Strategies builds the social services infrastructure for residents of public housing sites that are undergoing redevelopment into mixed-income communities. Urban Strategies understands that the physical revitalization of housing by itself is not sufficient to create communities which promote social and economic integration. In order to create successful and thriving mixed-income developments, a network of community support systems must be planned and implemented concurrent with the development of new housing. These supportive services offer public housing residents the opportunities to succeed and thrive in mixed–income communities.

JOB SUMMARY: The Senior Service Connectorleads a team of Service Connectors, with expertise in areas such as Social Work, case management, relocation and employment, to identify appropriate housing choices for families affected by redevelopment and to assist those families during and after housing transition. Support for families includes successful transitioning to new schools and to other services and opportunities in the new neighborhood. The Senior Service Connector utilizes knowledge of the San Francisco community, specifically Bay View, to establish effective relationships with businesses and social service organizations. The Senior Service Connector provides case management and referral and seamless coordination with other community and supportive services activities that promote economic self-sufficiency for families, enhance quality of life for seniors and persons with disabilities, and increase opportunities for residents as they prepare to live successfully in a new, mixed-income development or move toward housing self-sufficiency in the larger community. The Senior Service Connector supervises Service Connector staff, models professional behaviors necessary for community and economic development social work practice, and also works directly with residents as needed to support the efforts of the team.

DUTIES AND RESPONSIBILITIES:

1. Undertake and model aggressive and continuous outreach among residents to communicate to them the opportunities and requirements associated with the program and to promote participation by recruiting residents for appropriate activities.

2. Supervise members of the service connection staff and model professional social work practice.

3. Learn, fully utilize and prepare to support other staff members in the use of a client tracking system.
4. Through home and office visits meet regularly with targeted residents and establish and maintain working relationships with all heads-of-households and other adults between the ages of 19 and 64.

5. Perform initial and on-going assessment of each household. Develop realistic individual responsibility plans with residents between the ages of 19 and 64. Maintain frequent contact with members of the assigned case load.

6. Work with Urban Strategies, the City of San Francisco, San Francisco Housing Authority and other service provider staff to facilitate integrated services for residents and families as they prepare for self sufficiency.

7. Work with households to meet the requirements of re-occupancy of the new mixed-income development, including the requirements of the Economic Self-Reliance Agreement.

8. Provide ongoing coordination and follow-up of resource referrals for residents; monitor and document progress of residents enrolled in programs or services.

9. Advocate for residents and support them in meeting their needs; act as a liaison between residents and various stakeholder agencies, service providers and partners with an outcomes-oriented approach to achieving program goals.

10. Emphasize self-sufficiency through employment for persons between the ages of 19 and 64 by serving as job developer and job retention coach.

11. Emphasize the importance of high quality early childhood development programs for residents with young children in their households.

12. Emphasize the importance of participation in high quality youth development programs for residents with children or youth in their households.

13. Participate with the staff team to implement all activities set forth in the Service Plan for appropriate site.

14. Participate in site visits conducted by HUD, funders and other partners as requested by supervisory staff.

15. Lead and participate in weekly case conferences and provide regular caseload progress reports to Urban Strategies and other case management staff through the use of a client tracking program.

16. Attend and schedule case conferences with agencies or partners residents as needed. Participate in all required meetings, orientation, staff training, in-service and cross training opportunities.

17. Other duties as assigned.

QUALIFICATIONS:

Master’s degree or higher degree in Social Work or related field
Minimum of two years of supervisory experience and three years or more of field experience in working with low-income families is required; prior experience in community settings is preferred
Prior experience with program development and coordination is preferred
Commitment to strengths-based and family-focused service planning; ability to understand program philosophies and place-based approach
Must be highly skilled in basic case management functions such as assessment, goal planning, referrals and resource coordination
Must have ability to learn and effectively use custom client-tracking software to organize information and data related to assessments, referrals and follow-up of individuals
Must be able to multi-task easily and thrive in a fast-paced environment
Aptitude for developing rapport and effective working with clients in their home and community environment without compromising professional goals
Excellent communication skills, both in verbal and written form
Must be proficient in Microsoft Outlook, Word and Excel
