Case Manager With Car – Lamp Community – Los Angeles, CA

Lamp Community ends homelessness of Los Angeles’ most vulnerable individuals, primarily adults with mental illness, through a continuum of services and housing, enabling them to reach their highest level of self-sufficiency and community integration.

The RIST Case Manager will travel to multiple locations in the greater Los Angeles area to meet, transport, and work with members to attain and sustain permanent housing in partnership with the County Department of Health Services (DHS). The Advocate’s primary responsibilities are to outreach and engage new members and to coordinate housing, mental and primary health and recovery services that will ensure health and community integration.

ESSENTIAL DUTIES & RESPONSIBILITIES

1. *Transport members in your vehicle* as needed to assist members in accessing necessary services

2. In collaboration with DHS, outreach unsheltered and sheltered homeless who have chronic health conditions and/or are high utilizers of emergency county health services with the objective of engaging individual to determine motivation and eligibility for permanent housing

3. Complete pre-authorization and application package to submit to DHS per their policies and procedures

4. Assist engaged members with homeless section 8 voucher to find a safe place to live and contact Property Managers and Landlords to set up appointments to show members different apartment options

5. Travel to multiple housing locations in the greater Los Angeles area to provide case management; coordinate and implement mental health and recovery support services with individual members (caseload of 20-25)

6. Coordinate with DHS to ensure members are connected to primary health care and reduce need for emergency health care services

7. Develop effective, trusting relationships with members, with a focus on facilitating independence and maintenance of improved physical and mental health

8. Collaborate with each member to develop their individual service plan; review and update it quarterly and upon completion of goals

9. Assist management and other Advocates with program evaluation and modification for future program improvements and growth

10. Maintain a current, thorough knowledge of community resources and utilize them to provide comprehensive, wrap-around services to members

11. Assist members with becoming involved in daily activities, scheduling and attending appointments, budgeting, socialization, health maintenance, sense of community, and progress in recovery

12. Work cooperatively and cohesively with other members of the staff team, including participation in weekly staff meetings, group clinical supervision, and staff trainings

13. Develop and maintain a complete, accurate, and current member file with all required documents from DHS and HACLA

14. Assure that all member paperwork and data collection is complete and up-to-date in the Homeless Management Information System and case management software

QUALIFICATIONS

Current, valid California Driver’s with an acceptable driving record and reliable vehicle
Able to transport members in your vehicle
Bachelor’s degree and minimum two years’ experience working with homeless individuals, preferably those living with mental illness and/or substance addictions, or an equivalent combination of education and experience
Able to work with minimum supervision
Able to learn and utilize Exym database software
Detail oriented with excellent time management, organizational, written, verbal, and computer skills
Able to obtain and maintain CPR/1st Aid certification
Minimum 30 WPM typing speed preferred
