Job Description Residential Case Manager Hours: Fulltime weekends, evenings and overnight Job Site: Maternal Group Home and Transitional Living Program Reports To: Residential Program Coordinator Exemption Status: Non-Exempt
POSITION SUMMARY
The Residential Case Manager will be responsible for the supervision of the residents of the Maternal Group Home and Transitional Living Program on a daily basis.
RESPONSIBILITIES
Case Management • Supervise the activities of residents in the program • Conduct intake and orientation of new residents • Maintain an active client caseload • Provide residents with advocacy, referrals, crisis intervention and supportive counseling • Assist in developing and implementing appropriate service plans with residents • Ensure residents are following the guidelines and expectations of the program; giving consequences or discharging when appropriate • Monitor residents behaviors to ensure the safety of themselves and those around them •Facilitate psycho-educational and life skills groups
Administration • Assist residents in maintaining household standards of cleanliness • Coordinate recreational activities • Assist in the maintenance of program statistics and client records • Participate in weekly program staff meeting • Participate in monthly all staff meeting
Diversity • Ensure that all activities, services and programs through Bridge are conducted in a manner that is sensitive to and shows respect for the cultural and ethnic diversity for all Bridge clients
Other • Other related tasks as required/requested
SUPERVISORY REQUIREMENTS
• Weekly supervision with Residential Program Coordinator
QUALIFICATIONS
• BA Degree or related degree including supervised internship or • Two years experience working with adolescents • Previous experience in one or more of the following areas: house management, job development, community relations or recreational counseling • Continuous clean driver’s record • Valid Licenses as required
