Case Manager – Bronx – BronxWorks

Join a team committed to high quality services for people living with HIV/AIDS (PLWH/A) at BronxWorks, a community-based organization with over 40 years of experience in meeting needs in the Bronx! The Positive Living Department is seeking a case manager for its HIV/AIDS Case Management and Health Education Services contract. This contract focuses on PLWH/A who do not have Medicaid, including the undocumented. The case manager works with PLWH/A to assess and address their needs. The case manager makes referrals to meet the needs of clients and follows up on referrals to ensure connection to services. Responsibilities of the case manager include doing intakes, conducting assessments and developing service plans based on the assessment, working with the client to implement the service plan, conducting case conferences and home visits, and doing crisis intervention when needed.

B.A required, LMSW preferred. We will also consider candidates with a Masters in Social Work who become licensed within 6 months of being hired. The ideal candidate would have experience working with PLWH/A and with case management. Bilingual in English and Spanish preferred. Knowledge about reaching PLWH/A who do not have Medicaid, including the undocumented, preferred.
