Homes for the Homeless (HFH) provides transitional housing to homeless families with children in New York City. At our American Family Inns, families receive the education, job training, and support services necessary to maintain permanent housing and become self-sufficient. HFH seeks a two case managers for its South Bronx location.
The responsibilities of the Case Manager include, but are not limited to:
Handling a caseload of approximately 20 families
Developing service plans based on the assessment of clients’ needs and goals
Working with clients to maintain an open public assistance case
Assisting clients with referrals to outside agencies for employment assistance, medical services, GED assistance and follow-up ensuring they utilize all available resources
Maintaining and updating case files to document progress
Facilitating the flow of information to housing staff
Qualifications:
Bachelor’s degree in social sciences
Bilingual English/Spanish
Strong organizational skills and attention to detail
Working knowledge of Microsoft Office (Word and Excel)
Excellent written and verbal skills
