Case Manager – Huckleberry Youth Programs

RESPONSIBILITIES:
The primary responsibilities of this position are to insure that youth working with CARC receive caring and professional case management services; assist youth and their families with the implementation of recommended treatment plans so they are able to re-focus their attention into positive and productive behaviors and choices. The Case Managers must be able to work within collaborative process and negotiate successfully with multidisciplinary systems. A successful incumbent in this position is expected to operate automotive vehicles in the performance of assigned duties. Due to the nature of the assignment and the hours worked, public transportation may not be a cost effective or efficient method for traveling to the various locations required. The individual who is appointed to this position will be required to maintain a valid California Driver’s License throughout the tenure of employment OR demonstrate the ability to travel to various locations in a timely manner as required in the performance of duties. The Case Managers will be responsible for, but not limited to the following:

DUTIES:
Conduct assessments of clients to identify short/long term issues.
Develop, in conjunction with the CARC team, treatment goals and intervention
Facilitate meeting of treatment goals through the use of case management roles.
Network with other youth providers to establish liaisons and resources.
Maintain ongoing caseload and consistent documentation of clients.
Create client’s files.
Make official contacts with schools, counselors, P.O.’s, attorneys, and others associated with the youth.
Notify Probation Officers for those minors with court orders to comply in CARC programs and report on progress. Attend probation meetings & court dates.
Participate in training programs.
Conduct field visits on a weekly basis for clients on caseload.
Other duties as assigned.

QUALIFICATIONS:
A combination of three years of college and/or relevant experience working with the SFUSD or SF Juvenile Justice system preferred.
Knowledge of community based agencies serving adolescents in S.F strongly preferred
Past experience successfully working with disenfranchised youth strongly preferred.
Excellent organizational and writing skills required.
Demonstrate a creative, team-building, cooperative approach to job performance and seek to bring a constructive, problem-solving orientation to all tasks;
Knowledge of system collaboration preferred.
Criminal justice clearance as required by the agency.
Ability speak/read/write Spanish/English strongly preferred
Possession of reliable transportation, valid CA DL, insurance and clean DMV record.
Ability to lift & carry up to 10 pounds unassisted on a regular basis
