Case Manager – Project Hope Alliance – Costa Mesa, CA

The Case Manager will work closely with both the Education & Programs Manager and the Family Stability Program Manager to coordinate and provide services for unstably housed and homeless families and children, including advocacy and case management services. The Case Manager will work to maintain and strengthen the Education and Family Stability Programs and work in collaboration with staff and partners to provide comprehensive services to the children and families served by Project Hope Alliance (PHA). The case manager will provide direct education advocacy and rapid rehousing services to the families served by Project Hope Alliance. RESPONSIBILITIES / SPECIFIC JOB DUTIES

Complete an intake and needs assessment with each family which will be made up of a broad range of areas including: Income/employment/education, budgeting/credit, behavioral health, food/nutrition, children’s school/daycare, legal, health, parenting and daily living skills.

Works closely with families to identify and build upon strengths and develop strategies to address barriers and concerns identified through the assessment process.

Education Program:

Provide education advocacy and support to children, including the assessment and development of an action plan.
Make referrals and advocate for connections to education and community resources such as school and afterschool program placement, tutoring, counseling, etc.
Coordinate and facilitate Independent Education Plans and individualized educational support programs.
Maintain up-to-date files on individual children, activities, intakes, and exits
Work with parents to understand, formulate and execute education plans for their children.
Provide support and advocacy for children in school through the monitoring of school 
grades and attendance while partnering with Homeless Liaisons.
Hold case management meetings with parents to assess children’s needs and link families to appropriate resources.

Family Stability Program:

Develop a comprehensive case management plan, including financial planning and goals with individuals and families.
Assist participants with employment-related issues.
Develop and maintain detailed case files with charts and ongoing progress notes and outcome measures.
Recommend payment for rapid rehousing and complete paperwork per policy.
Maintain documentation of all payments to clients and all program expenses.
Collect and input client data in Vista Share in a timely manner.
Maintain client databases and referrals for services and generate reports.
Meet monthly with client’s individual and/or family unit, tracking progress towards goals.
Make appropriate referrals to the individuals/families needs.
Provide information and referrals for ancillary services.
Attend scheduled team meetings and trainings.
Perform other duties and special projects as required.

QUALIFICATIONS:

Bachelor’s degree preferred in Social Work, Human Services or related field and two years of directly related experience or an equivalent combination of education and experience.
Experience working with homeless families.
Knowledge of and sensitivity to issues of homelessness.
Ability to deal with crisis situations effectively and calmly .
Ability to be creative and flexible to accommodate immediate client and agency needs
Availability to work evenings and occasional weekends
Familiarity with education, community and social service agency/resources in the Orange County Area
Access to a car with current California driver’s license and automobile insurance
