SUMMARY

The HIPS Client Care and Quality Management Supervisor will serve as head of HIPS’ case management team. This position is a combination of direct service & supervision. The Client Care and Quality Management Supervisor will maintain a client caseload, provide street based & site based case management services, serve as the agencies HIPPA officer and track organizational referrals. The Client Care and Quality Management Supervisor will also serve as the day-to-day supervisor for the HIPS Case Management Team, ensuring proper implementation of the case management work plan developed in conjunction with the Health & Supportive Services Manager. This includes overseeing the development and implementation of case management policies & procedures, ensuring all case management data are accurately captured, providing case management policies and procedures training to all incoming case management staff, and overseeing the caseloads of case management staff under the Health and Supportive Services Manager’s supervision.

Responsibilities:

I   Program Support/Training

Maintains a client caseload in accordance with HIPS deliverables including providing mobile and site-based case management services.

Coordinate and facilitate monthly client case conference meeting with treatment cascade staff (Case Managers, CHW’s, TA, PrEP & SNS Specialists).

Oversee Testing procedures, PEMS submission and quality control, and help ensure HIPPA compliance across programming.

Ensure that all client referral and linkage requests are properly recorded, stored and communicated to the appropriate staff member(s).

Serve as first point-of-contact for all incoming outreach & hotline referrals, passing them on to appropriate staff. Supervise these referrals by: following up with any such referrals weekly through personal check-ins with staff members who are handling each referral, as well as periodic check-ins with clients to ensure that referrals are successful.

Solicit feedback from clients about the referral process and the results of the referral, and using this to create strategies for a more effective referral and linkage process.

Assist the Director of Programs in developing and updating client databases for on site, overnight outreach and syringe exchange clients, including a contact notes database to track progress and service outcomes of clients with complex needs.

Assist the Director of Programs with monitoring and evaluation, including analysis of program data and writing monthly reports on programs and service outcomes for clients.

Provides clinical consultation to staff.

Organizes and implements training program for staff (case managers and outreach workers).

Review client charts for all programs to insure compliance with HIPPA and Funder’s expectations and provide training on HIPPA documentation.

Provides back-up support to staff during scheduled or unscheduled absences of program staff.

Liaison and create partnerships with local private and public social service and mental health agencies.

II. External Education and Capacity Building:

Facilitate harm reduction, public health, or cultural competency workshops & trainings to be delivered to partner organizations and incoming volunteers.

Oversee Referral Guide and Case Management portions of the Program Manual, keeping it up to date.

Ensures all Case Management Team data & narratives are accurately recorded and delivered to Health & Supportive Services Manager by the 5th business day of the month.

Serves as the point person for vital partner collaborations in the areas of housing & housing policy advocacy, drug treatment & drug policy advocacy, violence prevention advocacy, advocacy with local law enforcement, and other harm reduction related service or policy partnerships.

Provide case management, referral, VD-SPDAT and CRCS systems training to incoming staff including GHC & AmeriCorps Case Manager.

Serves as the day-to-day supervisor of HIPS Case Management services and reports to the Health & Supportive Services Manager and involves them directly as needed.

Work with the Health and Supportive Services Manager to develop and implement more intensive service projects such as mobile case management and community resource and service linkage.

Work with the Capacity Building Community Resource Manager Assist in the facilitation of skills-based workshops quarterly for staff and volunteers. Workshops should focus on increasing knowledge of referral options for various issues (such as drug treatment, medical care, housing, etc), as well as skill-building around effective coordination of care and micro-case management during outreach and hotline services.

As head of HIPS’ case management team, assist in proposal development, identifying grant opportunities and contributing to program design that will increase organization’s housing and outreach capacity.

Participate in a monthly leadership meeting with members of Senior Mgmt. staff to report on and troubleshoot program successes/challenges/new developments.

III. Administrative Duties

Participates in the development and implementation of agency policies and practices affecting the overall operation and direction of the agency.

Assists with preparation/revision of agency program descriptions, promotional materials, etc. 10-10 10-10

Oversees Quality Assurance, ensuring program compliance with licensing regulations and HIPPA regulations .

Serves as the HIPPA Privacy Officer, ensuring compliance with HIPPA regulations.

Participates in 1-2 housing, drug tx, or other social service or advocacy committees of choosing.

Assist the Executive Director with preparing reports, grant applications, program proposals.

Provides presentations in community as assigned.

Additional Responsibilities

Attend weekly staff meetings, weekly one-on-one supervisions, monthly managers meetings, and monthly staff development days to share information and plan program work in team environment.

As part of a team, share in other organizational responsibilities as required including general administrative work, and volunteer training and representing HIPS to at least (2) community stakeholder groups.

Attend appropriate local, regional and national meetings to seek out current information about HIV prevention, treatment and care, and peer education and to disseminate information about HIPS programs to others

Qualifications and Skills Requirement

LSW Strongly Prefered. • Experience working with people who have severe and persistent mental illness required.The position requires knowledge of local community and national resources around housing, vital documents, drug tx and medical assistance as well as Harm Reduction, HIPPA regulations, HIV testing Protocols and case management.. This individual must work well [art of team and help to create a teamwork orientated atmosphere within the department. Experience developing policies and procedures, building collaborations, and advocacy strongly preferred. Experience working with sex workers, drug users, transgender individuals, harm-reduction philosophy, and anti-discrimination strategies • Advanced knowledge of evidence based treatment practices and methods required. • Experience in a leadership and supervisory role preferred. • Experience or knowledge of chemical dependency issues for adults with mental illness preferred. • Ability to intervene appropriately in crisis prevention and de-escalation when necessary. • Ability to communicate effectively and persuasively orally, in writing and on both an interpersonal and group level. • Knowledge and ability to facilitate groups. • Excellent interpersonal skills with people from differing backgrounds and cultures. • Ability to be flexible in meeting scheduling needs. Ability to work on agency holidays, as required. • Ability to maintain confidentiality of client information and appropriate boundaries. • Familiarity with Microsoft Office suite and Google Drive. WORKING CONDITIONS

This is a full time (40 Hours/week) salaried position with full benefits. Shifts will alternate at the HIPS Center for Health and Achievement 10am to 6pm M-F. Occasional off-site duties include attending shifts on the mobile, community partner meetings and HIPS community events. 1 day a month off-sit for visioning. Position is for 1 year, with the opportunity for an extended contract based on available funding. Some minimal lifting, physical activity, walking up stairs and sitting required.
