Opportunity:
In this role, you will be responsible for the supervision and direct oversight of a care management team serving clients who have chronic health and mental health conditions, guiding the team towards achieving goals and overseeing clinical assessments of clients (conducting assessments as needed). You will have significant decision-making, supervisory, administrative, and program management responsibilities and be critical in helping you team function at a high level to achieve performance targets, maintain Medicaid compliance, coordinate with Managed Care Organizations, and contribute to a culture that is client centered, outcome oriented, and dedicated to continuous quality improvement.

Qualifications:
The position requires Master’s Degree in Social Work, Mental Health Counseling, Public Health, Public Administration or related (LCSW preferred but not required) and 2+ years of applicable post-master’s experience. Excellent written and verbal communication skills coupled a collaborative work style based on open, transparent, continual communication, sharing, and inclusive decision-making are a must.
