Medicaid Service Coordinator (MSC) – New York Foundling Hospital

We are currently looking for a Medicaid Service Coordinator (MSC) to join our Bronx (IRA) Independent Residence Alternative Department.

The Medicaid Service Coordinator is responsible for providing services to 40-45 indivduals in several IRAs. Assist in obtaining needed services for the families of individuals on caseload. Ensure that residental, day habilitation, medical and all other needed services are provided to each individual as required by his/her Individual Service Plan. Meets face-to-face with each individual on caseload face at least three times annually and additionally as needed. Visits day program/work sites/schools as needed. Visit indivdual family homes as needed.

REQUIREMENTS: BA/BS required. MA/MS preferred. Experience with obtaining services/Medicaid Serivces required.
