Medical Case Manager – Downtown Women’s Center – Los Angeles, CA

The mission of the Downtown Women’s Center (DWC) is to provide permanent supportive housing and a safe and healthy community fostering dignity, respect, and personal stability, and to advocate ending homelessness for women. Founded in 1978, DWC was the first organization to exclusively serve mentally ill and elderly homeless women on Los Angeles’ Skid Row. The DWC Day Center now offers meals, case management, and other supportive resources on a drop-in basis to 4000 homeless women each year, while the DWC Residence provides a permanent home and a range of services to 119 women who were once without safe, affordable housing. Position Summary

The Downtown Women’s Center seeks a full-time Medical Case Manger to provide individualized care coordination and patient navigation services to homeless and formerly homeless women in the Skid Row community. Through personalized support and advocacy, this position works to connect women with medical and mental health services, and public benefits, as well as other services needed to support the client’s overall health and wellness. The Medical Case Manager is a member of the Housing and Clinical Health Services (HCHS) team and works as part of a group of clinicians and health professionals to empower women to achieve and maintain personal stability. This position reports to the Health Program Manager.

The Medical Case Manager plays a key role in the success of the DWC women’s health center. The position will provide direct services to clients with a focus on helping women access the services and treatment necessary to manage any physical and mental health issues. This position will collaborate with DWC staff and local service providers to connect clients to a medical home/primary care provider, health and other public benefits and ensure that they receive coordinated medical services. This position may: conduct intakes and assessments, schedule appointments, facilitate timely and appropriate access to health services through client support and advocacy, and assist with coordination of care plans.

Essential Functions

Provide individualized care coordination and patient navigation services; work with clients to create individual wellness plans that address their medical and mental health needs and goals; identify and provide needed resources and referrals
Guide clients through the health care system; assist with access to benefits and needed preventative, primary, and specialty care
Work to effectively resolve individual health barriers through timely follow-up, advocacy, relationships, and collaboration with DWC staff and other community service providers including hospitals, insurance providers, and other medical providers
Obtain updates and attend trainings regarding mainstream benefits and share with the team
Provide supportive counseling and crisis intervention as needed
Conduct outreach and public education activities to encourage client access to health services
Maintain accurate and timely records of activities, case management notes, and services provided to each client
Support the implementation of ongoing program evaluation and ensure adherence to reporting requirements
Provide regular, written reports to the Health Program Manager and additional reports as assigned
Offer guidance and coordination of interns and volunteers in the provision of services to clients
Perform other duties as assigned to support the mission of the organization

Necessary Skills/Qualifications

Master’s degree preferred (MSW, MPH, MA in related field); master’s degree in social work (MSW) strongly preferred
Two years experience working as a medical case manager/patient navigator/care coordinator with homeless, low-income and/or disabled individuals and knowledge of the medical and the public benefits system required
Knowledge of the complexity of poverty, women’s homelessness, health issues, mental illness, trauma, substance abuse and aging
Ability to problem solve and demonstrate an understanding of complex systems
Experience working in a medical, mental health, or public health setting
Excellent active listening and crisis intervention skills with the ability to exercise and model good judgment
Ability to manage multiple tasks and priorities; proficiency in balancing direct service responsibilities with effective record keeping
Ability to work well independently and with multiple teams while meeting the needs of both the clients and the organization
Strong administrative skills and proficiency in Microsoft Office Suite (Word, Excel, Outlook) required
Strong interpersonal, written, and oral communication skills; a proven ability to communicate with diverse audiences
Oral and written fluency in both Spanish and English is strongly preferred
Criminal background check and clearance required
Current valid California state driver’s license, insurance, and vehicle with good driving record

Schedule & Salary: This is a full-time, non-exempt position. Office hours are 8:30am to 5:00pm; with flexibility to work evenings and weekends when necessary. Salary range $35,000-$40,000 depending on qualifications & experience.
