Nurse Coordinator – SW Community Health Center – Portland, OR

Position: Nurse Coordinator

Reports to: Executive Director

Position Summary: Southwest Community Health Center (SWCHC) opened its doors in 2005 to provide quality health services to community members lacking adequate resources to access necessary care. SWCHC is at an exciting point in our history. The clinic expanded into Washington County last year and we are looking for two exceptional Nurse Coordinators to serve our growing patient population. These positions will work at both the SW Portland clinic and the new Hillsboro location.

Nurse Coordinators support SW Community Health Center’s mission of providing high quality health care in a safe, reassuring and culturally-sensitive manner by assuring smoothly-run clinics and continuity of care for all patients. This position is responsible for working with SWCHC’s Volunteer Coordinator to ensure that clinics are appropriately staffed and volunteers are properly oriented. In addition, Nurse Coordinators are responsible for ensuring that all necessary follow-up care is conducted and documented.

Typical Clinical Responsibilities: 1. Provide oversight during clinic hours, including: a. orchestration of clinic flow b. training and orientation of health care volunteers c. set-up and close-out for each clinic d. monitoring and assistance to clinicians e. provision of necessary lab work. 2. Return patient calls, as needed, or direct to the appropriate channel. 3. Assure follow-up care in accordance with clinic protocol and clinicians’ orders. 4. Provide chronic disease management services, including: a. identification of high risk patients b. monitoring of patients on SWCHC’s Chronic Disease Registries c. provision of follow-up for patients at-risk or missing their appointments d. provision of health education/health literacy information and support. 5. Oversee/manage the clinic’s Medical/Pharmacy Assistance Program. 6. Support patient outreach efforts. 7. Actively participate on the Clinic Operations Committee to establish and/or revise clinical policies and procedures. 8. Ensure that clinic labs are ready for courier pick-up and results are documented upon return. 9. Track and report clinical data, including immunization data. 10. Oversee volunteers providing quality control on clinic equipment, and assure that facility is properly sanitized after each clinic, including proper disposal of waste. 11. Provide other clinical duties as assigned.

Operational/Administrative Responsibilities (may include): 1. In accordance with the SWCHC Medical Director, establish protocols for the clinic in compliance with governmental guidelines and clinic policies. 2. Implement systems to help facilitate organizational flow. Create and/or update charts and forms in accordance with clinic protocol. 3. Work with HIT (Health Information Technology) Team to implement and utilize electronic medical records. 4. Sort and review incoming paperwork, including: labs, imaging, patient records and faxes. 5. Actively work with SWCHC’s HIPAA Office to assure compliance. 6. Work with Executive Director (ED) and Grants and Data Manager to implement, monitor and report on grant activities. 7. Manage Project Access, a program that provides no-cost specialty care and hospitalization for eligible clients. 8. Assist ED with community events, health fairs and promoting SWCHC.

Qualifications: 1. Current Oregon license as a Registered Nurse. 2. Three (3) or more years of experience in clinic-based nursing strongly preferred; 1-2 years required. 3. Excellent communication and interpersonal skills. 4. Bilingual skills – ability to read, write and speak fluently in Spanish and English – strongly preferred. 5. Willingness to continually strive to create a culturally-competent health care environment. 6. Ability to work with others to design efficient work-flow systems. 7. Excellent attention to detail and follow-through. 8. Ability to work under pressure and with interruptions. 9. Respect, kindness and compassion towards all SWCHC patients, staff and volunteers.

Physical Demands: While performing duties of this job, the employee is regularly required to sit and stand – sometimes for extended periods of time, communicate, and be able to operate a personal computer, printer, telephone and photocopier. Hours, Compensation and Benefits: This part-time position requires attendance at evening clinics (preparation and closure time included) as well as the provision of follow-up care on non-clinic days. Compensation will range from $24-$28/hour, DOE, with pro-rated benefits. The position may become full-time in the future, depending on SWCHC’s growth and employee’s availability.
