The New York City Health and Hospitals Corporation (HHC) is a $6.7 billion integrated healthcare delivery system with its own 420,000 member health plan, MetroPlus, and is the largest municipal healthcare organization in the country. HHC is committed to making exemplary healthcare accessible to all New Yorkers, and will be playing a crucial role in delivery system transformation over the next five years through its leading role in DSRIP, the Delivery System Reform Incentive Payment program. Through DSRIP, NYS DOH has allocated $6.42 Billion Medicaid dollars to fundamentally restructure the health care delivery system to transition care delivery from a largely inpatient-focused system to a community-facing system that addresses both medical needs and social determinants of health. DSRIP is a 5-year, performance payment-based program with primary goal of reducing avoidable hospital use by 25% over 5 years. At the end of program life, the aim is for the newly-transformed system to be sustainable.
HHC has formally partnered with hundreds of New York City provider and service organizations in order to achieve DSRIP transformation goals. This partnership is referred to by NYS DOH as a Performing Provider System (PPS), and the PPS led by HHC is called OneCity Health. HHC created a Central Services Organization (CSO) as a wholly-owned subsidiary of HHC, whose function is to provide operational, program, IT and other support to all OneCityHealth PPS partners and the OneCity Health governing bodies.
Position Description
The Project Manager will be responsible for managing the day-to-day implementation and operations of one or more OneCity Health programs or projects designed to meet DSRIP transformation goals, across partners who serve one or more boroughs in the city of New York. OneCity Health partners span the New York City, and the Project Manager will support one or more Hubs, which are defined geographic areas, usually boroughs. Program efforts are focused on achieving improved overall health through integration of behavioral health and primary care, provision of appropriate levels of care management, and care delivery models designed to improve chronic disease prevention and outcomes.
To support successful implementation, the Project Manager is expected to develop and oversee a defined set of project management, tracking, and reporting tools, and to establish effective working relationships with interdisciplinary clinical and operational teams across partner organizations. This role will require close coordination with Central Services Organization (“CSO”) functional areas, and the continuum of partners in a defined geographic area. This work must be accomplished in support of overall program goals and will contribute to OneCity Health’s ability to meet project milestones, including accurate and complete quality, process and financial reporting. S/he must understand both the DSRIP Program and local healthcare landscape in order to be successful.
This position reports directly to the OneCity Health Hub Executive Director.
Core Job Responsibilities
Developing and maintaining templates and tools to track project progress, drive decision making, and facilitate communication across the OneCity Health Hub. Tools may include status reports, dashboards, issue tracking and resolution lists, databases, distribution lists, and meeting facilitation materials.
Maintaining and updating work plans, tracking progress against plans, and identifying risks to successful project completion
Coordinating between CSO and Hub staff, including close collaboration with the other Hub Project Managers and HHC facility staff.
Advocating on behalf of the Hub within the central entities of the PPS.
Assist with management of day-to-day operations of DSRIP initiatives at implementation sites, working closely with and supporting Hub facility-based staff to facilitate timely and effective completion of deliverables required as part of the DSRIP Project Plan Application.
Assisting the Hub Executive Director in preparing for and running local implementation and operational meetings with HHC, community providers and partners, other PPSs, and external stakeholders.
Assisting in the development of necessary documents, including but not limited to performance reports and financial reports, which will be used by the CSO to complete quarterly reports required by the state.
Establishing and nurturing strong relationships between collaborating Performing Provider Systems (PPSs)
Education and Experience
Bachelor’s degree required; master’s in business administration (MBA), master’s in public health (MPH), or master’s in healthcare administration (MHA) is preferred.
5+ years of project or program management experience working within an integrated health system, health plan, ambulatory care setting or population health organization.
Experience with Brooklyn’s healthcare landscape.
Understanding of federal and state healthcare initiatives and regulations, including the DSRIP Program.
Experience with work plans, budgets, and performance and financial reporting.
Advanced skills in Microsoft Office applications including Excel, PowerPoint, and Word.
Creative problem solving skills.
Excellent written and oral presentation skills, with the ability to engage, inspire, build credibility and engender trust across all levels of an organization.
Ability to prioritize responsibilities, handle multiple projects simultaneously and delegate responsibilities when appropriate.
Ability to work well under the pressure of deadlines and the stress of undertaking multiple projects at one time.
Ability to make complex decisions with imperfect information.
