JOB ANNOUNCEMENT
REGISTERED NURSE CASE MANAGER
AIDS Project of the East Bay (APEB) is currently seeking a RN Case Manager to work with patients enrolled in our AIDS Medi-Cal Waiver Program. This program provides services for HIV positive patients who are living with advanced HIV disease and/or have an AIDS diagnosis.
I. JOB SUMMARY: The RN Case Manager provides assistance to clients/patients who are living with HIV/AIDS, particularly those who receive assistance from the Medi-Cal Waiver Program. The RN Case Manager must provide quality patient care through the development, implementation and evaluation of individual patient care plans. She/he will serve as a liaison between the agency, patient, and their families.

Reports to: Director of Operations
Clinical Supervision: Patient Care Manager
III. RESPONSIBILITIES AND DUTIES:
Assure that each client enrolled within the Case Management program meets the functional eligibility criteria by conducting an initial comprehensive nursing assessment of the client’s level of care and functional status. In addition, She/he will:
A. Consult with the client’s attending physician, primary care practitioner, social worker and other providers in order to coordinate plans of treatment.
B. Collect and maintain reporting data per State and Departmental requirements.
C. Develop individualized care plan for clients, including: emergency services, nutritional assessments, health care, HIV health education, referrals for emergency and permanent housing, mental health and substance abuse counseling and emotional and practical support.
D. Identify and follow-up on instances of abuse, neglect and exploitation that may bring harm or create potential harm to clients.
E. Empower and advocate for clients in decision making and health care service planning.
F. Monitor client progress as outlined in their individualized care plan, maintain an average of one successful contract per month, reassess client objectives and goals and make changes in accordance with the client’s changing needs.
G. Assist in developing outreach plans and activities that serve the homeless and other underserved populations within the program.
H. Participate in our weekly client service meeting, core case management team meetings and other department meetings, in order to consult with supervisors,
attending physicians and peers on case issues. Participate in annual in-service and trainings to receive updated information on services and entitlements.
I. Meet with clients in their place of residence in order to provide psychosocial support, assess needs, monitor progress and offer other supportive services.
J. Perform additional duties as assigned by the Director of Operations.
IV. QUALIFICATIONS:
A. Education: Registered Nurse licensed in the State of California with a degree from an accredited institution.
Certification: BLS, CNN, BSN required (PHN preferred)
B. Training and Experience:
1. Minimum 2 years’ experience, with a minimum of 2 years in the community nursing environment.
2. Knowledge of social service case management, and acute care to chronically ill clients; including practical knowledge of the social and economic aspects of clinical care.
3. Experience and ability to work with and possess sensitivity to the personal aspects of HIV disease.
4. Excellent verbal and written communication skills.
5. Experience working with communities of color, lesbian, transgender, gay and other underserved populations.
C. Special Requirements:
1. Bilingual in Spanish and/or experience working with the Hispanic population is a plus.
2. A vehicle, a valid California Driver’s License and proof of insurance.
3. May be exposed to unsanitary conditions in some home settings.
4. May be exposed to high crime areas within the service community.
5. May need to endure exposure to weather.
6. Ability to travel day to day within assigned geographic area
V. Compensation:
$63,000 – $72,000 annually; based on experience (Non-Exempt)
