Job description
The Jazz Foundation of America (JFA) is committed to providing jazz and blues musicians with financial, medical, housing, and legal assistance as well as performance opportunities, with a special focus on the elderly and veterans who have paid their dues and find themselves in crisis due to illness, age and/or circumstance. JFA achieves its mission through compassionate and personalized social work care that restores dignity and hope to their clients. JFA is saving jazz and blues “one musician at a time.”
The Jazz Foundation is seeking a full-time licensed social worker. LMSW required, LCSW preferred.
Must have at least three years of professional experience. Experience with the elderly preferred, and extensive knowledge of concrete services and state/federal entitlements such as Medicaid, Medicare, and Social Security needed. Love of jazz and blues preferred.
Duties include:
1-Psychosocial evaluations and current needs assessments.
2-Casework and group work for musicians dealing with medical, financial and legal crises.
3-Provide supportive counsel during times of crisis.
4-Meet with clients in the office and in their homes for additional assessment.
5-Referrals to medical, dental, housing and legal services and to other assistance organizations.
6-Regular case reviews with colleagues and partner organizations, to develop long-term solutions.
7-Extensive follow up with individual clients.
8-Maintain accurate and appropriate client records.
