Summary: The New Directions for Veterans SupportiveServices forVeterans Families(SSVF)program isarapidrehousing program designed toservevery low-income, primarily homeless Veteransand Veteran families.Theprogram assists Veterans inexiting homelessness topermanent housing and in maintaining apermanent housing placement.Servicesprovided by New Directions forVeteransincludesincludestreet and venue-based outreach,case management, permanent housing placement,and assistance in what ever benefitst he Veteran is eligible for. This program is a Housing First program that emphasizes Critical Time Intervention and Progressive Engagement,with the goal of(1)ensuring that no Veteran is ever forced to live on the street (2)ensuring that homelessness is rare and brief and that (3) there is a low percentage of veterans whore turn to homelessness after being served by New Directions for Veterans.
Responsibilities: The ND Vets Lead Case Manager will arrange, monitor, and deliver services related to meeting the needs of SSVF program veteran households while helping them obtain housing stability. The duties of the ND Vets SSVF Lead Case Manager duties include but are not limited to:
Conducts comprehensive screenings and assessments with veterans to collect functional, environmental, financial, employment, housing, educational, and health information, as appropriate, to develop a case plan
Provides service linkages and support systems for veterans and their families, including for children and adolescents, to ensure identified needs are met
Makes referrals to supportive services such as job developers, social workers and health care providers as deemed necessary
Develops support systems to meet veteran and family needs; coordinates and facilitates move-in activities, rental negotiations, and service referrals as required
May make home and field visits
Provide training, supervision, and problem-solving guidance for difficult cases
Perform case reviews and oversee appropriate use of community advocacy and wrap around services by Case Managers
Review check requests from other SSVF Cs Mgrs. and submit to supervisor for final approval.
Monitor data quality of the other SSVF Cs Mgrs.
Participate in street outreach to identify homeless veterans and present the program as an alternative to homelessness.
Liaison with the VA outreach departments, community outreach departments, VASH department and the VA benefits and hospital departments when applicable
Participate in conferences, workshops, special projects, staff meetings, and other duties as assigned
Requirements:
Master’s degree in a social services field strongly preferred, or Bachelor’s degree in a social service field from an accredited college or university
1 – 3 years’ experience working as a Case Manager with veterans, people in recovery and/or homeless individuals
Demonstrated proficiency in MS Word, Excel, and internet usage. HMIS a plus
Ability to develop relationships with families as well as a wide variety of service stakeholders (schools, health and public benefit agencies)
Effective written and oral communications skills
Some knowledge of affordable housing programs such Section 8 housing, VASH voucher and Shelter Plus Care programs
Experience and knowledge in working with a multidisciplinary treatment team
Additional Requirements:
US Armed Forces veteran preferred
Bilingual (Spanish) preferred
Able to lift up to 25 pounds
Valid California driver’s license, insurance and reliable transportation
