Supportive Case Manager

Seeking a Supportive Case Manager to work as a member of a team and provide community-based support services to approximately 20-25 emotionally disturbed children and their families; Services include assessment of need, care coordination, advocacy, entitlement counseling and linkages; home and school visits required. Must have High School diploma and 2 years experience working with children and their families; AA or BA may substitute for part of the experience; Spanish speaking preferred.
