§ Monitor the completion of all admission notes on each admitted resident.
§ Ensure that all necessary case notes and written reports are completed in a timely and concise manner by each of the staff.
§ Provide monthly individual educational/clinical sessions with case managers, reviewing charts and individual cases.
§ Provide daily direction to the Case Management staff, Activities Coordinator and Substance Abuse Counselor.
§ Provide bi-weekly group supervision with the clinical team (Nurse, Case Managers, Substance Abuse Counselor).
§ Communicate with family, collaterals, and other agency staff on behalf of residents as a part of service planning.
§ Schedule and conduct intake screenings.
§ Attend supervision sessions with the Program Director.
§ On call during off-hours and weekends, available for consultation on an as needed basis.
§ Participate in Utilization Review, and Discharge Planning.
§ Make professional visits where necessary and provide off-site screenings at other Agencies.
§ Maintain appropriate documents, records, and statistics and write reports in an organized, timely and accurate manner.
§ Attend off-site meetings as a representative of Unique People Services.
§ Provides emergency decision-making and crisis management.
§ Conduct unannounced visits to facility during off hours and weekends.
Qualifications
Master degree in Social Work, Social Service or Psychology and at least one (1) year experience working with homeless and mentally ill individuals; or a Bachelor degree in Human Service field and at least five (5) years working with persons diagnosed with a mental disability.
