Health Coordinator – Chicago – Howard Area Community Center

Summary of Job Description
Ensures accurate medical information in client files and provides approved resources, referrals, and follow-up to families in need of medical attention and assistance. Provides approvedinformation, support, and training to both staff and families involved in the Family Center Program, including Home-based and Center-based programs serving prenatal families and families with children 0-5 years old.

Sample of Specific Duties
Ensures accurate information in medical files through consultation with staff, file review, direct family contact, and interaction with community medical resources and providers.
Develops a tracking system and trains appropriate staff in its use for ensuring the accuracy and up-to-date nature of medical information regarding children and family members in each family file.
Develops and implements a communication system with staff and families regarding when medical, developmental, and other evaluations are due in order to ensure timely completion.
Weighs and measures children as necessary for file documentation.

Requirements & Qualifications
High School Diploma. Very strong interpersonal and organizational skills; knowledge of and skill in working with diverse populations; strong experience in web-based data management systems; strong presentation, public speaking, and facilitation skills; computer knowledge, including Excel, Microsoft Word, and use of the Internet are required. Also required are excellent writing and record-keeping skills and the ability to work independently as well as with a team. Knowledge of well child care and family medical needs a plus; experience working with families in poverty and at risk also a plus.
