NURSE PRACTITIONER – PEDIATRICS

(Exempt/Non-Union/Full-Time)

Salud Para La Gente (SPLG) provides high quality, comprehensive and cost-effective healthcare to underserved low-income communities in the Monterey Bay area, including Santa Cruz County and North Monterey County. We began in Watsonville in 1978 as a storefront “free clinic”. With the ever growing need for health services, in 1992 we became a federally qualified community health center. Today we are a primary health care network with 14 clinic sites and continue to provide high quality services to patients of all ages.

We are seeking a dynamic, organized and detail-oriented Associate Clinic Manager-Family Practice to provide basic medical care to clients, in accordance with established protocols. Care includes diagnosing, treating, and providing preventive medical services to primarily low-income Spanish-speaking farmworkers and their families. Adult, Pediatric, Prenatal care and services for chronic illness are also provided.

Responsibilities include, but are not limited to, the following:
Consistently adhere to and/or exceed SPLG’s communication guidelines and expectations with internal and external customers including co-workers, patients, and external partners.
Communicate effectively by using welcoming words, proper tone of voice, appropriate body language, eye contact and smiling in all interactions. Listen skillfully and display a willingness and ability to acknowledge patient and co-workers’ needs, expectations and values through the use of reflective listening and empathy conveyance. Respond to co-worker and patient needs in ways that are helpful and beyond expectation.
Performs direct patient care, including history-taking, physical examination, appropriate diagnostic procedures, therapy, and follow-up, under established protocols.
Communicate effectively and respectfully with people from different racial, ethnic, and cultural backgrounds and lifestyles, demonstrating a knowledge of and sensitivity to their needs.
Works in conjunction with the Clinic Manager, to insure optimal patient flow;
Works with scheduling staff to insure that advanced practitioner productivity standards of an average of 2.5 patients per patient care hour is achieved at a minimum.
Performs other medical services within the scope of Examining Committee guidelines under physician supervision.
Plans and initiates health maintenance and rehabilitation for each patient, including methods such as special diets, exercises, vitamin supplements, immunizations, etc., as needed.
Educates and counsels patient and/or family about normal developmental stages, health management, nutrition practices, chronic diseases, and medications, etc., intervening where appropriate.
Makes appropriate referrals to external and internal services for health and psychosocial needs, including follow-up to ensure patient compliance.
Institutes emergency measures in life-threatening situations.
May assist with the planning and staffing of off-site clinics, (i.e. Teen school programs, etc.)
In conjunction with Team Members of the Quality Improvement Committee, helps develop and update protocols for adolescent and other services and participates in peer review.
Becomes familiar with all clinical funding sources and contracted health plans; stays informed of patient care policies, reimbursement practices and reporting requirements mandated by funding sources; works with Clinic Business Manager, administrative and fiscal staff to insure that funding sources, clinical program objectives and targeted outcomes are achieved;
Required to document patient encounters utilizing Electronic Health Records (EHR) as well as completes encounter notes and billing through EHR as instructed by fiscal staff.
Participates in continuing education programs to remain current on professional development and practices and to maintain licensure.
Reviews policies, procedures and protocols within clinic manuals on a yearly basis; responsible to sign-off indicating that review/training has taken place.
Attends and participates in all agency staff meetings, medical management meetings, quality improvement meetings and provider meetings.
Maintains a valid professional license, updated CPR certificate and valid California drivers’ license; submits copies of renewed licenses and certificates to Human Resources.
At the request of the Clinic Manager may be requested to maintain a flexible schedule and to fill-in for other providers in the event of illness or vacation.
Works with the Board of Directors and administration towards meeting the goals of Salud Para La Gente.
Assists physicians with completion of electronic health record tasks and paperwork for patients.
Performs other duties as assigned.

MINIMUM QUALIFICATIONS
Fluent bilingual Spanish/English
Knowledge of Latino cultural medical practices and beliefs
Current PA/NP license
CPR certification
Willingness to work with HIV-infected clients.

DESIRABLE QUALIFICATIONS
Bi-cultural Spanish/English
