JOB OVERVIEW:
The Home Health Assessment Program (HHA) is designed to assist E-TANF recipients who are exempt from the E-TANF work requirement due to physical or behavioral conditions; have been repeatedly unsuccessful in obtaining and maintaining unsubsidized employment; have received benefits for four or more years and are unemployed. The Nurse Practitioner will report to the Assistant Director and will be responsible for in-home initial physical health assessments for all HHA clients. The Nurse Practitioner will meet with welfare recipients in their homes and determine the existence of physical health conditions, and then make recommendations about the recipients’ ability to work. As a Special Payroll employee, the Nurse Practitioner is not permitted to work more than 19 hours per week or 1,000 hours during the employment year and is not entitled to any PHMC benefits nor paid or unpaid leave.

RESPONSIBILITIES:
• Assess the presence, type, and severity of any physical health problems, in clients’ homes
• Assess the need for further evaluation by medical specialists
• Determine employment potential for each assessed recipient
• Collaborate closely with all members of project team, especially HHA Behavioral Health Assessors
• Produce written summaries of all assessments
• Attend all scheduled trainings and meetings
• Complete other duties as assigned

• Comply with Department of Public Health (DPH), The Joint Commission and other accreditation and regulatory agencies standards
• Adhere to all PHMC Policies and Procedures
• Knowledge and adherence to Infection Control and Environment of Care Guidelines and Procedures as described in the annual education module

• MSN

Licensure:
• CRNP as Adult, Family, Occupational Health or Rehabilitation Nurse Practitioner

PHYSICAL DEMANDS:
• Position requires standing 2/3 of the time, walking 2/3 of the time, requires sitting under 1/3 of the time, use of hands to finger, handle or feel 2/3 of the time, reach with hands and arms, under 1/3 of the time, stoop, kneel, crouch or crawl under 1/3 of the time, talk or hear over 2/3 of the time. Position requires lifting up to 10 lbs up to 1/3 of the time.

WORK ENVIRONMENT:
Moderate noise (examples: business office with computers and printers, light traffic)
