Purpose of the Position:
The Physical Therapist is a qualified professional person who directs, supervises, evaluates and provides physical therapy services to patients in the home as prescribed by the attending physician.
Essential Functions:
Assists the physician in evaluating patients by applying diagnostic and prognostic tests including muscles, nerve, joint and functional ability tests.
Conducts physical therapy assessments by medical order.
Identifies health care goals in collaboration with medical practitioner and the patient.
Identifies patient’s needs, develops problem lists and plans of care, progress notes and clinical summaries.
Identifies specific modalities of treatment, frequency of treatment and duration of treatment to meet patient health care goals.
Obtains medical orders for all treatments and services.
Makes accurate, consistent and concise entries in patient clinical record including treatment date, type, duration, response, and tolerance and patient progress at each visit.
Participates in the development and periodic revision of the physician’s treatment plan.
Collaborates with the patient’s health care team in meeting the patient’s health care goals.
Keeps accurate, concise, complete and timely records of patient care, and related information.
Arranges for outpatient services, which cannot be given in the home.
Develops client records and reports to the physician regarding the patient’s reaction to treatment and/or changes in the patient’s condition.
Instructs patients and their families in the use of prosthetic, orthic, and assistive devices.
Instructs paraprofessional staff regarding the patient’s physical therapy regime and use of assistive devices.
Participates in agency Quality Improvement Program as assigned or appointed.
Maintains close liaison with the agency to ensure that all clients receive coordinated, efficient, supervised and integrated care and that all members of the health care team have access to information regarding the client’s treatment and progress.
Knowledge, Skills and Abilities:
A minimum set of required skills will be identified that every graduate from a physical therapist assistant program can competently perform in the clinical environment.
Development of the minimum required skills will include, but not be limited to, the content blueprint for the physical therapist assistant licensure examination; no skills on the physical therapist assistant licensure blueprint will be excluded from the minimum required skills.
To achieve consensus on the minimum required skills, 90% or more of the member Consultants must agree.
The minimum required skill of the physical therapist assistant will not exceed that described for the physical therapist.
Those aspects of patient/client management that are not part of the scope of work of the physical therapist assistant are not addressed in this conference, i.e. examination,
Evaluation, diagnosis, prognosis, development of plan of care, re-examination, development of discharge plan.
Requirements:
Bachelor Degree in related field
One year of physical therapy experience or an equivalent combination of related education and experience
Licensed and currently registered to practice physical therapy under Title VII Article 136 of the New York State Education Law.
